Advantage Home Health is seeking a compassionate and dedicated Physical Therapist to join our growing Home Health team. If you're looking for the flexibility of a PRN schedule while making a real difference in patients' lives, this could be the perfect opportunity for you.

As a Home Health Physical Therapist, you'll enjoy the autonomy of managing your schedule, providing one-on-one care in patients' homes, and helping individuals regain strength, mobility, and independence where they are most comfortable.

Responsibilities

• Provide skilled physical therapy services in accordance with physician orders and individualized plans of care.


• Evaluate patients and develop personalized treatment programs to improve function and quality of life.


• Assess mobility, strength, balance, and safety needs.


• Document patient evaluations, treatments, and progress in a timely and accurate manner.


• Collaborate with physicians, nurses, and other healthcare professionals to ensure optimal patient outcomes.


• Educate patients and caregivers on exercises, safety techniques, and ongoing therapy goals.


• Supervise Physical Therapist Assistants as appropriate.

Qualifications

• Graduate of an accredited Physical Therapy program.
