Job summary

Frontpoint Health seeks a Home Health Physical Therapist (PT) to join our Dallas, TX team on a PRN per-visit basis.

You will assess patients in their homes, develop and implement treatment plans, and deliver therapy to restore mobility and independence while coordinating with physicians and care teams.

Qualifications

• Current license to practice as a Physical Therapist in Texas.
• Minimum of 1 year of home health experience.
• Strong communication and interpersonal skills.

Responsibilities

• Perform initial assessments and develop treatment plans.
