Job summary

BAYADA Home Health Care is seeking an experienced Physical Therapist (PT) for a per diem home health role serving Montgomery County, Maryland. You will provide in-home rehabilitative care to adult and geriatric clients, performing assessments, developing care plans, and coordinating with a multidisciplinary team.

This role welcomes new grads and offers flexible per-visit scheduling in a community-based office.

Qualifications

• Must hold a Maryland Physical Therapist license and graduate from an APTA-approved program.
• Ability to work independently and manage time effectively in home health settings.
• Strong interpersonal, organizational and problem-solving skills; familiarity with EMR is preferred.

Responsibilities

• Make home visits to clients in designated geographic territories.
• Perform diagnostic tests and measurements and develop care plans.
• Educate clients and family members on rehabilitative care and safety.
• Document observations and interventions in the EMR system.
