Premier Home Health Providers Inc. is seeking a per diem Physical Therapist to deliver skilled home health services in California. You will evaluate mobility, administer care per physician plans, and educate patients and families to support recovery in the home setting.

Requirements include a Bachelor’s or Master’s in Physical Therapy, active CA PT license, CPR (AHA), and 2+ years of PT experience with a home health background preferred. Flexible schedule and mileage reimbursement included.

Qualifications

• Evaluates patients’ functional status, mobility, gait, ROM to guide plans.
• Responsible for daily PT services per physician’s plan and California PT Act.
• Educates patients and families on exercises, equipment use, and recovery goals.

Responsabilités

• Evaluate patients’ functional status, mobility, gait, ROM to develop individualized treatment plans.
• Deliver physical therapy services per physician’s plan of care and CA PT Act.
• Educate patients and families or caregivers on therapeutic exercises, equipment use, and recovery goals.
• Collaborate with physicians and the care team—report changes in patient condition and participate in case conferences.
• Supervise PTAs and oversee Home Health Aide care bi-weekly when PT is sole skilled service.
• Manage documentation including progress summaries and discharge planning.
• Identify needs for additional home health services and coordinate referrals as appropriate.
