Job summary

Premier Home Health Providers Inc. is seeking a per diem Physical Therapist in California to assess and treat patients in home health settings. You will evaluate functional status, mobility, ROM, and gait, designing individualized plans of care in collaboration with physicians.

The role emphasizes patient education, safety, and coordination with care teams. Flexible schedule, potential for full-time conversion based on performance, and mileage reimbursement are offered.

Qualifications

• 2+ years of PT experience; home health background a plus.
• Active California Physical Therapy license.
• Current CPR certification with AHA.

Responsibilities

• Evaluate patients’ functional status, mobility, and ROM to develop treatment plans.
• Deliver PT services per physician plan and CA PT Act.
• Educate patients and families on exercises and home programs.
• Collaborate with physicians and care team; report changes and participate in case conferences.
• Supervise PTAs and oversee Home Health Aide care bi-weekly when PT is sole skilled staff.
• Manage clinical documentation and discharge planning.
• Identify needs for additional home health services and coordinate referrals.
