Job summary

Home Health Care, Inc. is seeking a licensed Physical Therapist (PT) to provide one-on-one home-based care across the Twin Cities. You will conduct comprehensive evaluations, develop personalized plans, and advance mobility in patients' homes with clinical autonomy and interdisciplinary support.

The role emphasizes independence, travel within the territory, and delivering care that improves functional independence in the patient's actual living environment.

Qualifications

• Must hold an active Minnesota PT license.
• Strong clinical reasoning and assessment skills.
• Excellent patient and caregiver communication.
• Willingness to travel within Twin Cities territory.

Responsibilities

• Complete comprehensive Physical Therapy evaluations in patients' homes.
• Develop individualized plans of care and functional goals.
• Improve mobility, strength, balance, endurance, transfers, and gait.
• Help reduce fall risk and improve home safety.
• Provide therapeutic exercise and functional mobility training.
• Educate patients and caregivers on home exercise programs and safe mobility techniques.
• Monitor patient progress and adjust treatment plans as needed.
• Collaborate with physicians, nurses, occupational therapists, speech therapists, and other interdisciplinary clinicians.
• Complete timely and accurate clinical documentation.
• Help patients safely regain independence and remain in their homes.
