Qualifications

• Active Minnesota Physical Therapist license (PT).
• Strong clinical reasoning and independent decision-making.
• Patience and passion for patient-centered care.
• Excellent communication and relationship-building abilities.
• Valid driver's license and reliable transportation.

Responsibilities

• Evaluate patients in their homes and develop individualized treatment plans.
• Improve strength, balance, mobility, endurance, and functional independence.
• Help patients safely navigate their home environments and prevent future injuries.
• Educate patients and caregivers to maximize long-term success.
• Collaborate with physicians and the interdisciplinary care team to achieve the best outcomes.
• Maintain timely and accurate clinical documentation.
