Access Home Health is seeking a Physical Therapist to provide one-on-one care in patients' homes, build relationships with clients, and manage a flexible schedule with strong clinical support. The role emphasizes independence while receiving responsive leadership and consistent referrals.

Full-time PT with Florida licensure, CPR, and solid communication skills will deliver evaluations, plan care, and document in WellSky, contributing to a high-quality home-health caseload and professional growth.

Qualifications

• Active Florida Physical Therapist License.
• Current CPR Certification.
• Strong communication and clinical decision-making skills.
• Ability to work independently.
• Home health experience preferred, but not required.

Responsibilities

• Perform evaluations and develop individualized treatment plans.
• Deliver skilled physical therapy services in patients' homes.
• Monitor patient progress and modify treatment plans as needed.
• Educate patients and caregivers.
• Collaborate with physicians and interdisciplinary team members.
• Complete documentation accurately and timely in WellSky (Kinnser).
