Job summary

Horizon Home Care & Hospice is seeking experienced Physical Therapists to join our Rehab team, delivering evaluation and therapy in clients' homes across Racine/Kenosha County. You will collaborate with an interdisciplinary team to restore function and prevent disability after illness or injury.

Qualifications

• Degree from a CAPTE-approved Physical Therapy Program.
• Current WI PT license.
• 1 year of therapy experience (Home Care preferred).
• Current/Valid WI Driver's License; reliable/insured vehicle.

Responsibilities

• Assess clients' physical, cognitive, behavioral, communication, emotional and social status.
• Assess home safety, medication management and fall risk.
• Submit OASIS data, physician orders and certifications per policy.
• Apply and modify skilled interventions based on patient needs and environment.
• Establish a plan of care with evidence-based interventions.
