Job summary

Horizon Home Care & Hospice, Inc. seeks a qualified Physical Therapist to provide skilled evaluation and therapy in clients' homes across Racine/Kenosha County, Wisconsin.

Work with an interdisciplinary team to restore function and prevent disability through patient-centered care. The role features a 1st shift schedule, no weekends or on-call, with competitive pay, mileage reimbursement, and comprehensive benefits.

Qualifications

• Must have a CAPTE-approved degree in Physical Therapy.
• Licensed as a Physical Therapist in Wisconsin.
• Minimum 1 year of therapy experience, home care preferred.
• Valid Wisconsin driver's license and own reliable vehicle.

Responsibilities

• Assess clients' physical, cognitive, behavioral, and social status in home settings.
• Develop Plan of Care and implement interventions based on current best practices.
• Collaborate with an interdisciplinary team and document OASIS/physician orders.
• Ensure safety, fall risk assessment, and home environment modifications as needed.
• Confer in regular team meetings to update care plans.

Education

Degree from CAPTE approved Physical Therapy Program

Current WI PT license

1 year of therapy experience (Home Care preferred)

Current WI Drivers License and vehicle
