Job summary

Atmore Community Home Care, part of LHC Group within Optum, is hiring a Physical Therapist in Home Health to assess patient needs, determine treatment plans, perform interventions, and evaluate progress. You will work with physicians and the health care team, educate patients and families, and document outcomes.

Requires current PT licensure in the state, a valid driver's license with reliable transportation, and CPR certification preferred within 90 days.

Qualifications

• Current Physical Therapy licensure in state of practice.
• Current driver's license and vehicle insurance, access to a dependable vehicle, or public transportation.
• CPR certification preferred within 90 days of hire.

Responsibilities

• Evaluates patient functional status and PT needs; assists physician in plan of care.
• Provides PT services within scope of practice per plan of care and state laws.
• Observes and reports patient responses to treatment to supervising nurse or physician.
• Treats patients to relieve pain, restore function, and maintain maximum functioning.
• Informs patient and family about plan of care, functional limitations and progress.
• Documents progress notes for each visit and completes PT plan of care within guidelines.
