Job summary

Addus HomeCare in Jackson, TN is seeking a compassionate Physical Therapist to join our Home Health team on a full-time basis. You will deliver PT services per a physician’s plan of care and collaborate with an interdisciplinary team to support patient independence in the home.

You will evaluate function, educate patients and families, and maintain accurate documentation while supervising assistants and ensuring high-quality, compliant care. Competitive pay and benefits offered.

Qualifications

• Graduation from an accredited Physical Therapy program (APTA) and current PT license in the state of practice.
• DPT or equivalent degree required and board certification preferred.
• CPR and First Aid certification is required.

Responsibilities

• Provide physical therapy services per physician's plan of care and state guidelines.
• Evaluate patient function and develop individualized care plans.
• Educate patients, families, and caregivers on treatment plans and home exercises.
• Translate exercises into functional activities and activities of daily living.
• Collaborate with physicians and interdisciplinary team to coordinate care.
• Document clinical notes, progress reports, and outcomes in a timely manner.
• Supervise Physical Therapist Assistants per policy and state regulations.
