Addus HomeCare in Clarksville, TN is seeking a dedicated Physical Therapist to join our Home Health team. You will provide therapy in patients' homes, help them recover, manage chronic illness, and maintain independence.

The role emphasizes patient education, collaboration with physicians and staff, and documentation of progress. We welcome candidates with licensure, CPR/First Aid, and at least two years' hospital experience seeking a meaningful community-based practice.

Qualifications

• Graduate from a School of Physical Therapy approved by the American Physical Therapy Association.
• Current licensure as a Physical Therapist in the state of practice.
• At least two (2) years' experience in an acute hospital or equivalent experience (preferred).
• Must possess current CPR and First Aid Certification.
• Must have reliable transportation, current driver's license and appropriate automobile insurance.

Responsibilities

• Provides physical therapy services to patients according to the physician's plan of care.
• Advise, consult and teach appropriate treatment procedures and supportive activities to clients and family members.
• Assist the provider in evaluating level of function.
• Participate in the development of the plan of care.
• Prepare clinical and progress notes.
• Translate exercises into functional activities or activities of daily living.
• Develop plans for modifying equipment and home surroundings as needed.
• Report client progress to attending provider and discuss factors affecting care.
• Explain treatment implications to clients and families.
• Provide information, support and motivation to help progress.
• Prepare and submit summaries of clinical progress.
• Supervise physical therapy assistants per policy and state regulations.
• Maintain accurate clinical records of goals and progress.
