Addus HomeCare in Jackson, TN is hiring a Physical Therapist Assistant (PTA) for full-time home health visits. You will provide direct PT services under a supervising PT, helping patients regain strength and independence in their homes.

The role emphasizes compassionate patient care, accurate documentation, and collaboration with a multidisciplinary team to support recovery and functional goals.

Qualifications

• Graduate of an accredited PTA program with an Associate’s degree approved by APTA.
• Current licensure as a Physical Therapist Assistant in the practicing state.
• At least 1 year of PTA experience preferred.
• CPR certification current and valid.
• Ability to make home visits and provide nursing care in home settings.
• Reliable transportation, valid driver's license, and auto insurance.

Responsibilities

• Provide direct PT services according to supervising PT's plan of care and state law.
• Treat patients to relieve pain, restore function, and maximize performance.
• Assist with exercises, gait training, and activities of daily living.
• Administer non-complex modalities as directed by the PT.
• Instruct and motivate patients on functional activities and home programs.
• Monitor progress and report findings to supervising PT and director.
• Prepare clinical notes and progress reports for the chart.
• Notify supervising PT of changes in patient condition and adjust plans as allowed.
• Instruct patients on use of assistive devices and prosthetics.
