Job summary

AccentCare seeks a Physical Therapist for Home Health in the Lehigh and Northampton counties area. This full-time role involves evaluation and treatment, working within an interdisciplinary team, and providing patient and family education.

You will manage a caseload, document per policies, and participate in conferences. Requirements include CAPTE-approved Master's in Physical Therapy, at least nine months of PT experience, and valid CPR certification; travel to all locations required.

Qualifications

• Master’s degree in physical therapy approved by CAPTE accredited program.
• Minimum nine months experience as a physical therapist.
• Licensed to practice as a physical therapist in the state of operation.
• Valid CPR certification required.
• Must be a licensed driver able to travel to all business locations.

Responsibilities

• Provides patient evaluation and treatment according to orders and policies.
• Maintains documentation and communicates with physicians and team members.
• Develops and documents treatment plans with measurable goals.
• Educates patients and families on procedures and care plans.
• Ensures productivity standards and efficient use of work hours.
• Participates in conferences and in-services as assigned.
