Job summary

Enhabit Home Health & Hospice is seeking a licensed Physical Therapist to provide patient-centered care in the home setting. You will assess, diagnose, and treat individuals with mobility impairments and functional limitations, using a range of physical modalities and specialty programs to improve independence and safety at home.

The ideal candidate has a valid PT license in the employment state and at least one year of clinical experience, plus a valid driver's license.

Qualifications

• Be currently licensed as a Physical Therapist (PT) in the state of employment.
• A minimum of one year of clinical experience is preferred.
• Demonstrate knowledge and skill in current therapy practice.
• Possess a valid state driver’s license and automobile liability insurance.

Responsibilities

• Examine and treat patients with physical impairments using physical modalities.
• Improve mobility and functional independence for daily living activities.
• Utilize Specialty Programs such as Balance and Fall Prevention, Spine Safety, Total Hip and Knee Replacement, and Total Shoulder Replacement to treat patients.
