CVHCare in San Ramon, CA, is seekingPhysical Therapists to join our Home Health care team. The PT will evaluate mobility, strength, and function in patients’ homes, establishing goals to improve balance, gait, and strength while educating patients and families.

Independence with support from an industry leader is emphasized. The role offers per diem scheduling with six visits per week and opportunities for non-traditional Thu-Mon schedules.

Qualifications

• Active California Physical Therapy license required.
• BLS certification required.
• Open to entry level or candidates with no prior home health experience.

Responsibilities

Schedule:Per Diem (6 visits p/week)

* Non-traditional schedules are available, i.e., Thursday-Monday

Area #44-La Canada, La Crescenta, Burbank, Montrose, Shadow Hills, Glendale, Sunland and Tujunga

About the position: Our Licensed Physical Therapists (PT) work in collaboration with the doctor, patient and the patient support system to provide best in class care for return to health and activity. As a Physical Therapist, you are responsible for the evaluation of mobility, strength and level of function. With this information you will establish goals to improve balance, walking, strength, as well as patient education. In the comfort of the patients’ home, the Physical Therapist assesses the home environment to ensure safe mobility is achieved. With your knowledge of best in class techniques, you will recommend equipment to enhance therapy, as well educate the patient on proper use.

In the unique setting of Home Health, each Physical Therapist sets their own schedule. You will find a great level of independence with the established support of an industry leader.

Requirements:

• Minimum one (1) year experience working as a Licensed Physical Therapist desired
• Open to entry level or candidates with no prior home health experience
• Current/active Physical Therapy License – State of California
• BLS Certification
• General knowledge and understanding of clinical verbiage
• Strong attention to detail for use in chart documentation
• Excellent time management/calendaring skills and ability to deliver documentation timely
• Strong experience using computers and computer systems
• Experience using Microsoft Suite (Word, Excel, Outlook), Gmail Suite or other systems
• Excellent written and verbal communication
