Job summary

Evolve Home Health in Agoura Hills, CA seeks Physical Therapists to provide home health PT across Los Angeles County and surrounding areas. The role involves following physician plans of care, evaluating patient needs, and traveling to patients' homes.

Candidates must have CA license, Bachelor’s or Master’s in Physical Therapy, CPR, and at least one year of home health experience. This position offers a flexible schedule and pay-per-visit compensation.

Qualifications

• Licensed to practice as a physical therapist in California.
• Minimum of 1 year of experience as a physical therapist (community/home health preferred).
• Excellent verbal and written communication and organizational skills.
• Current CPR certification.
• Valid Driver's License and reliable transportation.

Responsibilities

• Provide PT services to patients per physician's plan of care and home health requirements.
• Assess functional status, plan care, and evaluate home environment.
• Direct PT treatment and train patients/caregivers in therapeutic devices.
• Document progress and participate in discharge planning.
• Supervise PT assistants per policy; travel to patient homes as needed.

Job description

We are seekingPhysical Therapists (PT)with a background inhome health care experience.Applicants must haveprior experience in home health to be considered for this role.

Our agencyEvolve Home Healthis located in Agoura Hills, CA, we provide services throughout Los Angeles County and extending into the Mid Central Valley. (Fresno, Clovis, Madera, Coalinga, Mendota, Merced, Tulare, Visalia, San Diego area, and Los Angeles County).

ESSENTIAL JOB FUNCTIONS/RESPONSIBILITIES:

Responsible for providing physical therapy services and for adherence to all conditions outlined in the Personnel Services Agreement.

• Provides physical therapy services to patients according to a written physician's plan of care and as defined in the state Physical Therapy Practice Act. This may include, but will not be limited to:
  

A. Assessing and evaluating therapeutic/rehabilitative/functional status, and participation in the development of the total plan of care. Evaluates home environment.

B. Assesses for muscle strength, mobility, gait, ROM—potential for rehab.

C. Directing physical therapy treatment.

D. Instructing patients and families/caregivers in the use and care of therapeutic appliances.

E. Determining priority needs for physical therapy.

F. Reporting to physician patient’s reaction to treatment or changes in condition.

• Initiates physical therapy program and instructs other personnel and/or family/caregiver members in certain phases of physical therapy with which they may work with a patient, as well as instructing them as to the goals of the physical therapy program for the patient by participating in case conferences.
• May train patient in the use of prosthetic device.
• Identifies patient and family/caregiver needs for other home health services and refers as necessary.
• Prepares and submits clinical and progress summaries based on the attainment of goals.
• Participates in discharge planning for patient.
• Prepares and submits a clinical progress summary based on the attainment of goals as directed by Organization policy.
• Provides physical therapy consultation to home families/caregivers when indicated.
• Provides in-service education programs for nursing organization personnel as needed.
• Participates in peer consultation process.
• Supervises physical therapy assistants according to organization policy and state regulations.
• When therapy is the only skilled service, instructs, supervises and evaluates home health aide care every two (2) weeks.

POSITION QUALIFICATIONS:

1. Possesses a degree from a baccalaureate or masters program in physical therapy approved by an accredited organization. Meets personnel qualifications stated in Home Health Conditions of Participation§484.115(h).

2. Licensed to practice as physical therapist by the Physical Therapy Board of California.

3. One year of appropriate experience as a physical therapist. Community/home health experience is preferred.

4. Demonstrates good verbal and written communication and organization skills.

5. Possesses and maintains current CPR certification.

6.Physical Exam, TB Test, Chest X-ray

7. Valid Drivers License, reliable mode of transportation, and proof of insurance

OTHER REQUIREMENTS:

• Willing to travel up to 100% of the time for business purposes
• Flexibility to travel within assigned serviced areas

Pay: $100.00 - $120.00 per hour *Pay per visit
