Live Well Homecare Agency, LLC is seeking a Full-Time Physical Therapist to join a home health care team, enabling patients to recover at home. You will perform assessments, develop and implement treatment plans, and document patient progress, delivering therapy services per physician orders.

Responsibilities include evaluating mobility, creating plans of care, discharge planning, and supervising PT Assistants and Home Health Aides where applicable, with OASIS documentation and strict compliance

Qualifications

• Degree in Physical Therapy from an accredited program.
• Unrestricted state license as a Physical Therapist.
• Valid CPR certification.
