Job summary

Human Touch Home Health is seeking Physical Therapists to provide home-based PT services across Hyattsville and Northern Prince George’s County. Choose full-time, part-time, or PRN schedules with flexible territories and dedicated clinical leadership.

You will evaluate and treat patients at home, develop plans of care, and coordinate with physicians. Home health experience is preferred but not required; WellSky documentation is used for patient progress.

Qualifications

• Active Maryland license required for PT practice.
• Graduate of an accredited Physical Therapy program.
• Reliable transportation to travel to patient homes.
• Strong clinical and communication skills for home health.
• Ability to independently manage home health visits.
• Excellent organizational and time-management abilities.
• Home Health Experience preferred but not required.
• WellSky experience is a plus.

Responsibilities

• Complete comprehensive physical therapy evaluations.
• Develop individualized plans of care.
• Assess strength, balance, mobility, gait, endurance, and functional limitations.
• Provide therapeutic exercise and functional mobility training.
• Provide gait, transfer, and stair training.
• Address fall risks and home-safety concerns.
• Develop and reinforce home exercise programs.
• Recommend appropriate assistive devices when indicated.
• Educate patients, families, and caregivers.
• Monitor progress and adjust treatment plans as appropriate.
• Coordinate care with physicians and the interdisciplinary team.
• Complete reassessments and discharge planning.
• Complete accurate and timely documentation using WellSky.
