Human Touch Home Health is hiring Physical Therapists for full-time, part-time, and PRN home health positions in Germantown, Damascus, Cabin John, and surrounding Montgomery County, MD. You will deliver one-on-one in-home PT, develop plans of care, and help patients regain mobility with a focus on safety and independence.

Qualifications

• Active Maryland Physical Therapist License Required.
• Graduate of an accredited Physical Therapy program.
• Reliable transportation to patient homes.
• Strong clinical and communication skills.
• Home health experience preferred, but not required.
• WellSky experience is a plus.

Responsabilités

• Complete physical therapy evaluations in patients' homes.
• Develop individualized plans of care.
• Help patients improve strength, balance, endurance, and mobility.
• Provide therapeutic exercise and functional training.
• Provide gait, transfer, and stair training.
• Assess fall risks and home safety.
• Educate patients, families, and caregivers.
• Monitor patient progress and modify treatment plans.
• Complete reassessments and discharge planning.
• Collaborate with the interdisciplinary care team.
• Complete timely clinical documentation using WellSky.
