Job summary

Human Touch Home Health in Maryland is seeking a compassionate Physical Therapist (PT) to provide skilled home health therapy to patients in Prince George’s County and Montgomery County. You will perform evaluations, develop plans of care, and deliver interventions to improve mobility, balance and independence in patients’ homes, coordinating with an interdisciplinary team.

Qualifications

• Current and unrestricted Physical Therapist license in Maryland.
• Graduate of an accredited Physical Therapy program.
• Home health experience preferred, but qualified candidates from other clinical settings are encouraged to apply.
• Strong clinical assessment and patient-care skills.
• Excellent communication, organization, and time-management skills.
• Ability to travel throughout Prince George’s County and Montgomery County.

Responsibilities

• Perform comprehensive physical therapy evaluations in patients’ homes
• Develop individualized plans of care based on each patient’s needs and goals
• Provide skilled physical therapy interventions to improve mobility, strength, balance, endurance, and overall functional independence
• Evaluate patients for fall risks and home safety concerns
• Educate patients and caregivers on exercises, mobility techniques, transfer training, and safety
• Recommend appropriate assistive devices and durable medical equipment when needed
• Monitor patient progress and modify treatment plans as appropriate
• Coordinate care with physicians, nurses, occupational therapists, speech therapists, and other members of the interdisciplinary team
• Complete all required clinical documentation accurately and within required timeframes
• Communicate significant changes in patient condition to the appropriate members of the care team
• Follow agency policies and all applicable state, federal, and Medicare requirements

Human Touch Home Health is seeking a compassionate and motivated Physical Therapist (PT) to provide skilled home health therapy services to patients throughout Prince George’s County and Montgomery County, Maryland.

Key Responsibilities
