Job summary

Human Touch Home Health is seeking a compassionate Physical Therapist (PT) to provide skilled home health therapy services across Prince George’s County and Montgomery County, Maryland.

You will perform home-based evaluations, develop individualized care plans, and implement interventions to improve mobility and independence, while educating patients and caregivers and coordinating with an interdisciplinary team in a patient-centered environment.

Qualifications

• Current and unrestricted Physical Therapist license in Maryland.
• Graduate of an accredited Physical Therapy program.
• Home health experience preferred, but qualified candidates from other settings are encouraged to apply.
• Strong clinical assessment and patient-care skills.
• Excellent communication, organization, and time-management skills.
• Ability to work independently in a field-based environment.
• Reliable transportation and valid driver’s license.
• Ability to travel throughout Prince George’s County and Montgomery County

Responsibilities

• Perform comprehensive physical therapy evaluations in patients’ homes.
• Develop individualized plans of care based on each patient’s needs and goals.
• Provide skilled physical therapy interventions to improve mobility, strength, balance, endurance, and overall functional independence.
• Evaluate patients for fall risks and home safety concerns.
• Educate patients and caregivers on exercises, mobility techniques, transfer training, and safety.
• Recommend appropriate assistive devices and durable medical equipment when needed.
• Monitor patient progress and modify treatment plans as appropriate.
• Coordinate care with physicians, nurses, occupational therapists, speech therapists, and other members of the interdisciplinary team.
• Complete all required clinical documentation accurately and within required timeframes.
• Communicate significant changes in patient condition to the appropriate members of the care team.
• Follow agency policies and all applicable state, federal, and Medicare requirements
