Cherokee Nation Home Health Service is seeking a Home Health Physical Therapist to provide in-home therapy services in Tahlequah, OK. You will assess patients, design personalized treatment plans, and work with physicians, nurses, and caregivers to support functional recovery.

The role emphasizes collaboration, safety, and outreach to improve mobility, pain management, and independence in patients' homes. Oklahoma licensure and CPR certification are required.

Qualifications

• Current Oklahoma license as a Physical Therapist.
• Graduate of an accredited Physical Therapy program.
• Home health experience preferred but not required.
• Current CPR certification.
• Valid driver's license, reliable transportation, and automobile insurance.
• Strong communication, organization, and time-management skills.

Responsibilities

• Evaluate patients' physical abilities and develop individualized treatment plans.
• Provide therapeutic exercises, gait training, balance activities, and mobility interventions.
• Assess home environments and recommend equipment or modifications to improve safety and independence.
• Educate patients and caregivers on treatment plans, exercises, and injury prevention strategies.
• Document patient progress and maintain accurate clinical records in compliance with agency and regulatory standards.
• Collaborate with interdisciplinary team members to coordinate care and achieve optimal patient outcomes.
• Monitor patient responses to treatment and modify care plans as needed.

In the role of the Home Health Physical Therapist (PT), you will provide therapy services to patients in their homes, helping them regain mobility, improve functional independence, manage pain, and enhance their overall quality of life.
