Job summary

Cherokee Nation Home Health Service in Stilwell, OK is seeking a licensed Physical Therapist to provide in-home therapy across Stilwell and surrounding areas, including Westville and Watts-Westville. The role focuses on evaluating patients, developing personalized plans, and promoting independence within patients’ homes.

The PT will collaborate with an interdisciplinary team, educate patients and caregivers, and document progress to ensure quality care.

Qualifications

• Current Oklahoma license as a Physical Therapist.
• Graduate of an accredited Physical Therapy program.
• Home health experience preferred but not required.
• Current CPR certification.
• Valid driver’s license, reliable transportation, and automobile insurance.
• Strong communication, organization, and time-management skills.

Responsibilities

• Evaluate patients' physical abilities and develop individualized treatment plans.
• Provide therapeutic exercises, gait training, balance activities, and mobility interventions.
• Assess home environments and recommend equipment or modifications to improve safety and independence.
• Educate patients and caregivers on treatment plans, exercises, and injury prevention strategies.
• Document patient progress and maintain accurate clinical records in compliance with agency and regulatory standards.
• Collaborate with interdisciplinary team members to coordinate care and achieve optimal patient outcomes.
• Monitor patient responses to treatment and modify care plans as needed.
