PRN Home Health & Therapy, a Wisconsin-based family-owned company, is seeking a Physical Therapist for in-home care. This 0.8-1.0 FTE role emphasizes flexibility, autonomy, and making a meaningful impact in patients' lives.

You'll perform in-home evaluations, develop individualized treatment plans, and collaborate with patients, families, and the care team. The position includes supervision of PTAs, progress documentation, and patient/caregiver education.

Qualifications

• Current Wisconsin Physical Therapist license.
• Valid driver’s license and reliable transportation.
• Home health experience preferred, but not required.
• Strong communication and compassionate patient care.

Responsibilities

• Complete in-home evaluations and create individualized treatment plans.
• Provide evidence-based therapy to improve mobility and function.
• Collaborate with patients, families, and the care team.
• Document patient progress and outcomes.
• Supervise Physical Therapist Assistants (PTAs).
• Educate patients and caregivers on safety and mobility.
