Frontpoint Health is hiring a committed Physical Therapist for home health in Longview, TX. You will assess patients, help develop care plans, and guide in exercises, transfers and ADL training to restore function in patients’ homes.

Qualifications

• Current state license as a Physical Therapist.
• Graduate of an American Physical Therapy Association approved physical therapy program.
• Minimum of 1 year of experience in home health.
• Strong communication and interpersonal skills.
• Ability to work independently and as part of a team.
• Excellent time management and organizational skills.
• Valid driver's license and reliable transportation.

Responsibilities

• Conduct patient assessments and assist physician in evaluating function.
• Assist in development of the Plan of Care.
• Direct and aid patients in exercise, gait, ADL, transfer and gait training.
• Treat patients to relieve pain and restore function using exercise, massage, heat, and electrical modalities.
• Observe and report patient responses to treatment to supervisor.
• Instruct patients and caregivers in use of equipment and care strategies.
• Visit patients per Plan of Treatment; document progress notes.
• Prepare discharge summaries and periodic condition reports every 60 days.
