Qualifications

• Graduated from CAPTE-accredited PT program; DPT preferred.
• Current Texas PT license and CPR certification.
• Valid Texas driver’s license and reliable transportation.

Responsibilities

• Perform comprehensive PT evaluations and develop treatment plans.
• Provide skilled PT services per physician orders and regulations.
• Educate patients and caregivers on exercises and safety.
• Document all care accurately in the EMR system.
• Coordinate with physicians and the interdisciplinary team.
• Travel to patients' homes within service areas.
