All Care and Comcare Home Health is seeking a compassionate Physical Therapist to provide in-home therapy across our service area. You will perform SOC/ROC/Recert/DC evaluations and tailor plans to individual patient goals, collaborating with an interdisciplinary team.

The role requires CA PT licensure, CPR, and willingness to travel to patient homes. Excellent communication and organizational skills are essential in this flexible, patient-centered setting.

Qualifications

• Active California Physical Therapy license.
• Graduate of an accredited Physical Therapy program.
• Strong documentation and organizational skills.
• Excellent communication and interpersonal abilities.
• Willingness to travel to patient homes for evaluations and treatment.
• Flexible regarding work locations across service area.

Responsabilités

• Provide comprehensive, skilled physical therapy services and interventions per physician orders.
• Perform SOC, ROC, Recert, and DC evaluations.
• Assess patient needs, plan interventions, evaluate outcomes, and modify therapy goals.
• Collaborate with the interdisciplinary team to develop patient goals.
• Educate patients, families, and caregivers on PT programs and home exercise plans.
