Job summary

All Care and Comcare Home Health is seeking a compassionate Physical Therapist to join our home health team in California. This role offers a flexible schedule and the opportunity to provide individualized care in patients' homes alongside a supportive team.

You will perform SOC/ROC/Recert and discharge processes, collaborate with an interdisciplinary team, educate patients and families, and travel across the service area to deliver high-quality, goal-oriented therapy.

Qualifications

• Active California Physical Therapy license (PT)
• Graduate of an accredited Physical Therapy program
• Current CPR certification
• Valid California driver’s license
• Current California auto insurance
• Completion of physical exam, TB test, and required vaccines/titers
• Two professional references

Responsibilities

• Provide comprehensive, skilled physical therapy services and interventions following physician orders.
• Perform SOC evaluations, ROC, recertifications, and discharges.
• Assess patient needs, plan interventions, evaluate outcomes, and modify care to meet PT goals.
• Collaborate with the interdisciplinary team to set and evaluate individualized patient goals.
• Educate and train patients, families, and caregivers on home exercise programs.
