Job summary

All Care and Comcare Home Health is seeking a compassionate Physical Therapist to provide skilled PT services in patients' homes across our service area. You will perform SOC, ROC, recertifications, and discharges, while developing individualized goals with the interdisciplinary team.

Requirements include CA PT license, CPR, driver's license and auto insurance, plus strong documentation and communication skills.

Qualifications

• Active license/registration with the Physical Therapy Board of California is required.
• Strong documentation and organizational skills are essential.
• Excellent communication and interpersonal abilities are required.
• Willingness to travel to patient homes for evaluations and treatment.
• Flexibility regarding work locations across the service area is needed.

Responsibilities

• Provide comprehensive, skilled physical therapy services and interventions in accordance with physician orders.
• Perform SOC, ROC, recertifications, and discharges.
• Assess patient needs, plan interventions, evaluate outcomes, and modify care to meet goals.
• Collaborate with the interdisciplinary team to develop and evaluate patient goals.
• Educate, instruct, and train patients, families, and caregivers on PT programs and home exercises.
