Qualifications

• Doctor of Physical Therapy degree from an accredited program.
• Current licensure as a Physical Therapist in Virginia.
• Strong communication and interpersonal skills.
• Ability to work independently and as part of a team.
• Proven track record of delivering effective home health care.

Responsibilities

• Conduct comprehensive PT assessments and develop personalized treatment plans.
• Provide one-on-one PT interventions, exercises, modalities, and caregiver education.
• Collaborate with physicians, nurses, and social workers for comprehensive care plans.
• Maintain accurate and timely documentation of patient progress and billing information.
• Develop strategies to prevent hospital readmissions and improve outcomes.
• Stay up-to-date with evidence-based practices and guidelines.
