Qualifications

• Graduation from an approved Physical Therapy curriculum.
• Virginia Physical Therapist license (active) required.
• Experience in home health preferred.
• Strong clinical assessment, organizational and communication skills.
• Bilingual in Spanish preferred.

Responsibilities

• Develop and adjust a conditioning/rehabilitation plan of care with client, caregiver(s), and physician(s).
• Test and screen client strength to assist physician evaluations.
• Educate clients and families on exercise programs and therapies.
• Document observations, interventions and evaluations accurately.
• Provide direct patient care including evaluation and treatment planning.
• Review referrals and medical records to determine PT treatment needs.
• Record prognosis, treatment and progress in charts or electronic systems.
• Set goals and reevaluate plans as needed.
• Evaluate treatment effects and adjust plans for maximum benefit.
• Participate in service educational activities and clinical studies.
• Perform other duties as assigned.
