Classic Home Care, a nurse-owned and operated skilled home health agency in Southeast Michigan since 2003, seeks dedicated Physical Therapists for home health. Our Northville office supports a team delivering high-quality care to patients across surrounding communities.

This contingent PT role offers flexible hours with potential to transition to full-time or part-time caseload; competitive per-visit rates, mileage reimbursement, and a path for advancement in an RN-led environment.

Qualifications

• Active Michigan Physical Therapy license
• Reliable transportation
• Valid driver\'s license and auto insurance in your name as a driver
• Home health care experience preferred

Responsibilities

• Provides skilled physical therapy in accordance with the physician’s plan of care.
• Assists the physician in assessing the patient’s functional level and helps develop the plan of care including OASIS assessments and goals.
• Treats patients to relieve pain, restore function and maximize performance using PT treatments and modalities as stated in the plan of care.
• Observes, records and reports the patient’s reaction to treatment and any change in their condition to the physician and/or care team.
• Instructs patient, family and health team personnel in phases of physical therapy and proper use of equipment to promote independence.
