Job summary

Boost Home Healthcare – Northern Colorado is seeking a licensed Physical Therapist to join our Greeley team. The role is full-time, with a day shift Monday through Friday in the home health setting, weekends as needed, and a competitive per-visit pay of $68–$76.

You will assess patients, design plans of care, complete OASIS documentation, and collaborate with an interdisciplinary team to support recovery and independence in the patients' homes.

Qualifications

• Graduate with a Bachelors, Masters, or Doctorate in Physical Therapy from an accredited program.
• Active Physical Therapist (PT) license in the state of practice.
• Minimum of 1 year of experience in a home health setting preferred.
• Proficient with Medicare OASIS documentation and clinical care planning.
• Mindset: dedication to serving others and improving patient quality of life.

Responsibilities

• Deliver physician-prescribed physical therapy to help patients regain independence.
• Perform comprehensive initial assessments, develop customized plans of care, create maintenance programs, and conduct 30-day reassessments.
• Complete accurate Medicare OASIS documentation in a timely manner.
• Collaborate with interdisciplinary home care team members during patient care planning.
• Direct and supervise clinical personnel as required.
