Job summary

Boost Home Healthcare in Greeley, CO seeks a full-time Physical Therapist (PT) to join our home-based care team. You will deliver physician-prescribed PT services in patients' homes, perform initial assessments, create care plans, and conduct 30-day re-assessments while documenting Medicare OASIS.

Qualifications

Graduate with a PT degree from CAPTE/APTA-accredited program.
Active PT license in state of practice.
Minimum 1 year of home health experience preferred.
Proficient with Medicare OASIS documentation and care planning.

Responsibilities

Deliver physician-prescribed physical therapy to help patients regain independence.
Perform initial assessments and develop and adjust plans of care.
Complete Medicare OASIS documentation in a timely manner.
Collaborate with interdisciplinary home care team members.
Direct and supervise clinical personnel as required.
