Job summary

US Healthcare Consulting Holdings LLC is seeking a Physical Therapist (PT) for home health in the Northern Colorado Loveland/Fort Collins area. This full-time role offers competitive pay and a flexible schedule within a growing organization focused on patient-centered care.

You will evaluate patients at home, develop plans of care, provide therapy to improve mobility and safety, and collaborate with the care team. Colorado licensure or eligibility is required.

Qualifications

• Current Colorado Physical Therapist license or eligibility for Colorado licensure.
• Graduate of an accredited Physical Therapy program.
• Current driver's license and reliable transportation.
• Excellent communication and organizational skills.
• Ability to work independently and manage a patient caseload.
• Strong commitment to patient-centered care.
• Home health experience is preferred, but not required.
• Experience with geriatric or post-acute patients is a plus.

Responsibilities

• Provide skilled physical therapy services to patients in their homes.
• Develop individualized plans of care based on patient needs and goals.
• Improve mobility, strength, balance, endurance, and independence.
• Assess home safety and recommend equipment or modifications.
• Educate patients, families, and caregivers.
• Monitor progress and modify treatment plans as needed.
• Collaborate with physicians, nurses, therapists and the team.
• Complete clinical documentation in a timely manner.
• Participate in care coordination and discharge planning.
