Job summary

Boost Home Healthcare in Greeley, CO seeks a full-time Physical Therapist (PT) to join our home-based care team. You will deliver physician-prescribed PT services in patients' homes, perform initial assessments, create care plans, and conduct 30-day re-assessments while documenting Medicare OASIS.

We offer a flexible schedule, competitive pay per visit, and opportunities for advancement. A bachelor/master/doctorate in PT with an active license is required, with at least one year of home health

• Active PT license in state of practice.

Responsibilities

• Complete Medicare OASIS documentation in a timely manner.
• Collaborate with interdisciplinary home care team members.
• Direct and supervise clinical personnel as required.

Physical Therapist (PT) – Full-Time

Location: Greeley, Colorado (and nearby areas)

Schedule: Monday – Friday, Day Shift (Weekends as needed)

Pay: $68.00 – $76.00 per visit

Key Responsibilities

As a Home Health Physical Therapist (PT), you will:

• Deliver physician-prescribed physical therapy to help patients minimize physical disabilities and regain optimal independence.
• Perform comprehensive initial assessments, develop customized plans of care, create maintenance programs, and conduct 30-day reassessments.
• Complete accurate Medicare OASIS documentation in a timely manner.
• Collaborate with interdisciplinary home care team members during patient care planning.
• Direct and supervise clinical personnel as required.

Requirements & Qualifications

• Education: Graduate with a Bachelor’s, Master’s, or Doctorate in Physical Therapy from an accredited program (CAPTE / APTA).
• Licensure: Active Physical Therapist (PT) license in the state of practice.
• Experience: Minimum of 1 year of experience in a home health setting preferred.
• Skills: Proficient with Medicare OASIS documentation and clinical care planning.
