Job summary

Stern at Home Therapy is seeking Physical Therapists to provide one-on-one home health care for a geriatric population in the Long Branch, NJ area. Flexible part-time or full-time roles are available through W-2, 1099, and salary options.

Full-time 1099 tiered roles offer high earning potential up to $145,000 annually depending on visit volume; other structures provide competitive per-visit rates and benefits. A NJ license and independent work capability are required.

Qualifications

• Active Physical Therapist license in New Jersey (NJ).
• Home health experience preferred.
• Reliable transportation required.
• Ability to work independently in a home-based setting.

Responsibilities

• Perform physical therapy evaluations and develop individualized plans of care.
• Provide one-on-one treatments based on each patient’s functional needs and goals.
• Manage treatment plans with clinical autonomy.
• Help patients improve mobility, strength, balance, safety and function.
• Complete accurate and timely clinical documentation.
• Communicate with patients, families, physicians, and the clinical team.
• Coordinate scheduling, patient care and logistics with administration.
