Qualifications

• Active PT license in New Jersey (required).
• Home health experience preferred - not required.
• Strong communication and bedside manner.

Responsibilities

• Evaluate and treat a geriatric population in patients' homes around Jersey City.
• Develop and implement individualized treatment plans with clinical autonomy.
• Coordinate care with an admin team and ensure timely documentation.
