Job summary

Graham Healthcare Group is seeking a Home Health Physical Therapy Assistant for a role based in Lyons, Illinois. The PTA will collaborate to provide management and delivery of patient care plans within a home health setting.

The position requires an Associate degree in physical therapy, PTA license, and a valid driver’s license with travel to patient locations. The role includes a comprehensive benefits package and a strong company support system.

Qualifications

• Associate degree in physical therapy from an accredited college or technical school required.
• Minimum one year of experience in an acute care and/or rehabilitative setting preferred; previous home health experience preferred.
• Must hold, in good standing, a license/registration as a Physical Therapist Assistant.
• Current driver’s license and ability to spend ~20% of your day driving to/from patient locations.
• Commitment to meet critical deadlines for charting.
• Experience with tablets, mobile phones and EMR software.

Responsibilities

• Collaborate to provide management and delivery of patient care plans.
• Support patients in designated service area.
• Maintain charting and EMR updates as required.
