Job summary

Premier Home Health Providers Inc. seeks a dedicated Physical Therapist to deliver skilled services in patients' homes across California. This per diem role supports recovery, mobility, and independence with compassionate care.

You will evaluate functional status, develop plans of care, educate families, and coordinate with physicians, with opportunities to convert to full-time based on performance. A valid CA PT license and CPR are required, along with a reliable vehicle.

Qualifications

• Strong communication skills with patients and families.

• Ability to coordinate with care teams and report changes.

Responsibilities

• Evaluate patients’ functional status, mobility, gait, and ROM to develop individualized treatment plans.
• Deliver physical therapy services per physician’s plan of care and CA PT Practice Act.
• Educate patients and caregivers on exercises, equipment use, and recovery goals.
• Collaborate with physicians and care team; report changes and participate in case conferences.
• Supervise PT Assistants and oversee Home Health Aide care bi-weekly when PT is primary skilled service.
• Manage and complete documentation including progress summaries and discharge planning.
• Identify needs for additional home health services and coordinate referrals.
