Job summary

Premier Home Health is seeking a per diem Physical Therapist in California to deliver skilled home health services. You will evaluate patients, develop individualized plans, and work with physicians to execute the treatment plan, ensuring safe and effective care in patients’ homes.

Role emphasizes collaboration with care teams, caregiver education, and comprehensive documentation. A strong clinical background, teamwork, and empathy are essential to support recovery and improve quality of life

Qualifications

• Bachelor’s or Master’s degree in Physical Therapy from an accredited program.
• Active California Physical Therapy license.
• Current CPR certification with AHA.
• Strong communication skills and the ability to connect with patients and families.
• Valid driver’s license and insured vehicle.

Responsibilities

• Evaluate patients’ functional status, mobility, muscle strength, gait, and ROM to develop individualized treatment plans.
• Deliver physical therapy services in accordance with the physician’s plan of care and California’s Physical Therapy Practice Act.
• Educate patients and their families or caregivers on therapeutic exercises, equipment use, and recovery goals.
• Collaborate with physicians and the care team—reporting changes in patient condition and participating in case conferences.
• Supervise Physical Therapy Assistants and, when PT is the sole skilled service, oversee Home Health Aide care bi-weekly.
• Manage documentation including clinical progress summaries and discharge planning.
• Identify needs for additional home health services and coordinate referrals as appropriate.
• Perform other duties as required for patient services.
