Premier Home Health Providers Inc. seeks a per diem Physical Therapist to deliver skilled care in patients' homes across California. You will evaluate mobility, design plans of care, and work with a multidisciplinary team to promote recovery and independence.

The role requires active CA PT license, 2+ years in PT (home health a plus), CPR (AHA), and reliable transportation. This is a flexible, patient-focused position with potential for full-time conversion.

Qualifikationen

• Bachelor's or Master's degree in Physical Therapy from an accredited program.
• Active California Physical Therapy license.
• CPR certification (AHA) is current and valid.
• Valid driver's license and reliable vehicle.

Aufgaben

• Evaluate patients' functional status, mobility, muscle strength, gait, and ROM to create individualized plans.
• Deliver PT services per physician's plan of care and CA PT Practice Act.
• Educate patients and families on exercises, equipment use, and recovery goals.
• Collaborate with physicians and care team; report changes and participate in case conferences.
