Job summary

CareCo Medical Home Health is seeking a licensed Physical Therapist to provide skilled in-home therapy services to patients in the community. You will plan and deliver individualized therapies in patients' homes, collaborate with the care team, and educate patients and caregivers to promote recovery and safety.

The role involves independent work with a per visit schedule, mileage reimbursement, and ongoing professional development opportunities in a supportive interdisciplinary setting.

Qualifications

• Current Physical Therapist license in Connecticut.
• Minimum of three years of healthcare experience.
• Home health and OASIS experience preferred.
• Strong communication and documentation skills.
• Ability to work independently with minimal supervision.
• Valid driver's license and reliable vehicle.

Responsibilities

• Perform physical therapy evaluations, assessments, and in-home treatments.
• Complete initial and comprehensive assessments including OASIS within required timeframes.
• Develop, implement, and update Plans of Care.
