Qualifications

• Active Physical Therapy licensure in the state of practice.
• Valid driver's license and vehicle access or reliable transportation.
• Current CPR certification or ability to obtain within 90 days of hire.

Responsibilities

• Evaluates the patient’s functional status and physical therapy needs; assists physician in plan of care.
• Provides services within the scope of practice and in coordination with the health care team.
• Observes and reports patient response to treatment and changes in condition to the supervising nurse or physician.
• Treats patients to relieve pain, restore function, and maintain maximum functioning.
• Instructs patients, families, and care teams about plan of care, limitations, and progress toward goals.
• Visits patients per Plan of Treatment and submits progress notes per policy.
• Completes evaluations and develops the PT plan of care within state guidelines.
• Reports outcomes of evaluation, goals, and anticipated frequency of care.
• Function in diverse home settings respecting all patient demographics.
