KingRecruitingServices is seeking a dedicated Physical Therapist for a full-time home health role in Denver. The PT will provide patient-centered care in patients’ homes, document treatments, and coordinate with physicians and the care team to meet goals.

The position requires a CAPTE-accredited degree, current state licensure, CPR certification, and a valid driver’s license for travel between locations. Community/home health experience is preferred.

Formación

• Master’s degree in physical therapy from a CAPTE-accredited program.
• Currently licensed to practice as a physical therapist in the operation state.
• Valid CPR certification.
• Must possess and maintain a valid driver’s license to travel to all locations.

Responsabilidades

• Provides patient care and documents treatment per physician orders and agency policy.
• Performs patient evaluations and selects appropriate diagnostic materials.
• Informs physician of assessment results and recommended interventions.
• Develops and documents treatment plans with measurable goals.
• Educates and counsels patients and supports family members.
• Presents patient information to physicians and keeps the team informed of status and plan.
• Delivers treatment within agency and payer guidelines.
• Sets objective, measurable goals in collaboration with the team and patient.
• Meets productivity standards and manages work hours efficiently.
