Job summary

Brookestone Home Health Care is seeking a compassionate Physical Therapist (PT) for part-time or PRN work in the Omaha area. You will evaluate and implement personalized plans to improve mobility and independence in the home setting, collaborating with an interdisciplinary team to deliver patient-centered care.

Responsibilities include evaluations, customized treatment plans, gait/balance training, caregiver education, and timely documentation while adhering to regulations and agency policies.

Qualifications

• Nebraska Physical Therapist license in good standing.
• CPR certification.
• Valid driver's license, reliable transportation, and proof of automobile insurance.
• Ability to travel within the service area.

Responsibilities

• Perform comprehensive physical therapy evaluations and reassessments.
• Design and implement individualized treatment programs.
• Provide therapeutic exercises, gait and balance training, transfer training, and fall prevention interventions.
• Educate patients and caregivers on home exercise programs and treatment strategies.
• Collaborate with physicians, nurses and other health professionals to coordinate care.
