Job summary

Brookestone Home Health Care in Omaha, NE is seeking a compassionate Physical Therapist (PT) for part-time or PRN work, offering flexible scheduling and the opportunity to provide personalized home-based care to patients and families in the Omaha area.

The PT will evaluate, develop, and implement treatment plans to improve strength, balance, mobility, and independence, working with an interdisciplinary team to deliver high-quality patient-centered care in the home setting.

Qualifications

• Active Nebraska Physical Therapist license in good standing.
• Current CPR certification.
• Valid driver's license, reliable transportation, and proof of automobile insurance.
• Ability to travel within the service area.

Responsibilities

• Perform comprehensive physical therapy evaluations and reassessments.
• Design and implement individualized treatment programs.
• Provide therapeutic exercises, gait and balance training, transfer training, and fall prevention.
• Recommend and assess the need for assistive devices and durable medical equipment.
• Educate patients and caregivers on treatment strategies and home exercise programs.
• Track progress and adjust treatment plans to achieve optimal outcomes.
• Collaborate with physicians, nurses, and other healthcare professionals to coordinate care.
• Complete accurate and timely clinical documentation.
• Adhere to agency policies, professional standards, and regulatory requirements.

Position Overview
