Job summary

Brookestone Home Health & Hospice is seeking a compassionate Home Health Physical Therapist to join our growth-focused team in the Kearney area. This part-time and PRN role focuses on one-on-one patient care, developing and implementing personalized treatment plans to improve mobility, function, and independence at home.

You'll work with patients, families, and an interdisciplinary team to promote safety, quality of life, and patient-centered outcomes.

Brookestone Home Health & Hospice -- Kearney, a proud member of the Vetter Senior Living family, is seeking a compassionate and dedicated Physical Therapist to join our growing team. We provide personalized home health and hospice services designed to help patients improve mobility, regain function, and maintain independence in the comfort of their own homes.

As a Home Health Physical Therapist, you'll have the opportunity to make a meaningful impact through one-on-one patient care. Working closely with patients, families, and an interdisciplinary care team, you'll develop and implement individualized treatment plans that promote safety, independence, and quality of life.
